2.1.3 Medical Entrance Requirements for Students

Policy

Pursuant to state law (110 ILCS 20/), all students born on or after January 1, 1957 who are enrolled in an on-campus class(es) are required to provide proof of immunity according to law (against tetanus, diphtheria, rubella, mumps, and measles). Documentation of immunity must be complete and on file at Student Health Services before the 10th class day of the first term enrolled to avoid a registration block and late processing fee for the registration block to be lifted.

Proof of Immunity May be Documented By:

1. A copy of the student's Illinois high school health record which complies with the immunization requirements; or
2. Other acceptable medical record signed by a physician or public health official showing required immunization and immunization dates.

Proof of Immunity Will Not Be Required If:

1. The student's instruction solely involves research, field work or study outside of a classroom environment (no contact with other students).
2. A healthcare provider certifies that an immunization(s) is medically contraindicated (Note: If a student's condition later changes to permit immunization, he/she shall be required to obtain the immunization(s) from which he/she had been exempted). OR
3. The student submits a written statement detailing his/her objection to immunization based upon religious grounds. General philosophical or moral objection to immunization shall not be sufficient for an exemption on religious grounds.
