Pursuant to 5 ILCS 420/4A-101, some employees of Illinois State University may be required to file Statements of Economic Interests with the Office of the Secretary of State.
The following criteria are used to determine what Illinois State University employees will be required to file an annual Statement of Economic Interests:
On an annual basis, Human Resources will identify the Illinois State University employees whose job descriptions meet the annual statutory filing criteria for the Statements of Economic Interests.
In March of each year, the Office of the Secretary of State Index Division will send notification letters and forms to the home addresses of the Illinois State University employees who have been identified as required to file a Statement under the Illinois State Employees and Officials Ethics Act.
Employees required to file must return the original completed and signed Statement of Economic Interests form to the University Ethics Office, no later than April 23rd of each year.
Upon receipt of the employee’s Statement, the University Ethics Office will review the Statement, submit the original copy to the Office of the Secretary of State on or before May 1st of each year as required by law, and send each employee a confirmation letter noting receipt of the completed Statement and the date the original Statement was filed with the Secretary of State.
Please note that after April 23rd, employees who have not yet filed their Statements should mail their Statement directly to the Office of the Secretary of State Index Division and provide the University Ethics Officer with a photocopy of the original.
Penalties are assessed by the Office of the Secretary of State to those who file after the May 1st deadline as follows:
A $15.00 fee for failing to meet the May 1st statutory filing deadline.
A $100 per day fee (in addition to the $15.00 fee) from May 16th until the date of the Statement is filed with the Secretary of State Index Division.
Failure to file by May 31st filing deadline will result in the Secretary of State Index Division providing the name of the non-compliant employees to the Office of the Attorney General for discipline including the possible forfeiture of employment.
All employees required to complete a Statement of Economic Interests based on the criteria identified in the Illinois Governmental Ethics Act (5 ILCS 420/Art.4A-101) must also complete the Supplemental Statement of Interests. This Supplemental Statement is required by Ill. Executive Order 15-09 (January 13, 2015).
An electronic copy of the supplemental state of economic interest form will be emailed to each employee via their ilstu.edu account and may be submitted electronically to the Executive Ethics Commission.
